Please arrive about 10 minutes early to allow time to fill out a New Patient form. If you prefer, you can download the form below, complete it at home, and bring it to your appointment.

During your first visit, your dentist will review your medical and dental history and perform a thorough examination. This includes evaluating your teeth, gums, bite, and jaw, as well as taking any necessary X-rays to provide a complete picture of your oral health.
Treatment Plan
After your examination, your dentist will create a personalized treatment plan based on your unique oral health care needs. They will also take the time to discuss your options and answer any questions you may have.

At Noftall Family Dentistry, we'll help you navigate our options for payment, insurance, and financing so you can focus on achieving your dental goals.
Insurance Claims
We can direct bill your insurance company and our administrative team is happy to assist you in understanding your insurance plan better.
Payment Methods
The full treatment cost (or the patient portion if direct-billing to your insurance) must be paid at the time of your appointment. We accept payment by cash, debit or most major credit cards.
